Hi Everyone,

Adam’s family has been left with hundreds of thousands of dollars in medical bills. I know many of you have been wondering what you could do to directly help the family, which is why we have chosen to pin cookrocks54’s information.

All of the information I’ve listed below can be found in her post or by visiting cookrocks54’s site, fanssupportadamcook.com

At this time there are several options available, the family has approved all of these and I’m sure they would appreciate your generosity in any amount, no matter how small.

Please see the list of available options below, again you can also find all of this information in cookrocks54’s post. I just thought this format might make it a little easier for you to view all the current options available. If the family approves more options we will add them to this list.

I would also like to emphasize that the money all goes to the family, whether you donate to either bank account, or purchase a bracelet or t-shirt.

I’d like to thank cookrocks54 for her fundraising efforts over the past year, including collecting money for Adam to attend last years American Idol finale. Also I’d like to say THANK YOU to everyone who has already donated to 1 or more of these options either in the past or more recently.

There are currently 2 bank accounts set up to collect funds for the Cook Family.

The Adam N. Cook Fund
This account is held at Old National Bank in Terre Haute, IN. The account was established last year and is a general fund which is available to the Cook Family to access for whatever their needs may be.

The bank was unable to set up a credit card account for an individual, so instead a Paypal account has been established for those who would like to make a contribution online. Old National Bank Officer, Vickie Wallace transfers all funds from the paypal account directly to the Adam N. Cook Fund. The Cook Family Trust Fund
This account was recently established at First Financial Bank in Terre Haute, IN. This account is a Trust Fund to benefit the Cook Family. Again, contributing to either account is fine and both the Trust Fund and General Fund fully support the existence of each other.

At this time credit card payments are not accepted.

AC Guitar T-Shirts
The t-shirts design was created by cookrocks54 and Ho, and was drawn by Ho. All proceeds are donated directly to Adam Cook’s family to help pay his medical bills. The cost is $29.99 per shirt U.S. buyers, $32.99 for Canada and $34.99 for all other countries. This price includes postage within the U.S. and a rough estimate of international postage charges; any excess funds will be donated to the Cook family.

Adam Cook/Grey Matters Bracelets
Cookrocks54 has received permission from Julie Henricks, from Terre Haute, IN radio station 102.7 to sell the Adam Cook/Grey Matters bracelets via Paypal because the radio station can’t accept credit cards or online orders directly. The cost is $9.00 for each bracelet, which includes delivery confirmation and shipping costs within the U.S. and $9.00 for international orders, which also includes shipping. International orders can’t be tracked or insured at this time. Please allow 4-6 weeks for delivery of bracelets.
